Climate commitments and health policy integration: A decade review of COP and G20 actions toward One Health alignment

Methods:
We conducted a systematic review of official outcomes from COP21 to COP29 and G20 documents related to climate and health. Health-related content was identified and coded to assess frequency and the extent to which documents offered actionable guidance for urban/rural planning, national policy, or health system resilience. A mixed-methods approach was applied, combining quantitative frequency analysis with qualitative thematic coding.
Results:
We observed a notable shift in health integration beginning with COP28 (2023), which featured the first Health Day and the UAE Declaration on Climate and Health. WHO’s COP29 report further detailed actionable strategies aligned with One Health, including sustainable transport, green urban design, and decarbonized health systems. The 2024 G20 Health Ministers’ Declaration marked the first direct linkage between One Health, climate, and equity. Prior G20 climate communiqués largely omitted health or implementation guidance. These developments illustrate a growing commitment to health within global climate discourse, though gaps remain in policy application.
Implications:
The growing recognition of health as a strategic element of climate governance must now be matched by robust implementation tools. Embedding health into Nationally Determined Contributions (NDCs), fostering intersectoral coordination, and securing sustainable financing are essential for operationalizing public health goals. This review offers a timely framework for transforming international commitments into practical, equity-driven strategies that build health and climate resilience at national and local levels.
