A Population Based Case Control Study of Ethnic Differences in the Utilization of Elective Hip or Knee Replacement Surgery for Arthritis

This project examines the utilization of elective hip or knee replacements for arthritis among Hispanics and non-Hispanics in Bexar County, Texas. The researcher directly assess persons hospitalized for these procedures between February 1999 and January 2000. Three lines of analysis are being pursued. First, a case control study is being conducted comparing the ethnic background of recipients of an elective arthritis-related hip or knee replacement surgery against the ethnic background of age and gender matched controls hospitalized for other reasons. Second, the PI is developing population-based utilization rates for elective hip or knee replacement surgery using census derived demographic information as the denominator population. Particular attention is being placed on the ethnic background, acculturation, socioeconomic status (SES) and clinical characteristics of persons receiving these procedures. Finally, the completeness of their population -based estimates will be assessed using Medicare and Medicaid claims data to measure the extent to which Bexar County residents undergo hip or knee replacement in hospitals outside their county of residence. The project is a population based case control study. Odds ratio of Hispanic ethnicity will be calculated for hip replacements and for knee replacements, compared to controls. Logistic regression model will be use to measure the likelihood of Hispanic ethnicity among recipients of an elective hip or knee replacement (SES, acculturation).

Learning Objectives: 1. Compare ethnic backgrounds between persons hospitalized for elective arthritis-related hip/knee replacement surgery and persons hospitalized for other reasons. 2. Discuss the association between SES and acculturation and the likelihood of recipients of hip/knee replacement being Hispanics compared to others. 3. Assess to what extent Bexar County, Texas residents who are Medicare and Medicaid beneficiaries undergo elective hip/knee replacement for arthritis outside the county
