This paper describes a component of the evaluation of the Kansas LEAN School Health Project, specifically, changes in intermediate outcomes (i.e., new or modified programs, policies or practices facilitated by project staff and volunteers) resulting from project efforts. The paper presents a working hypothesis linking intermediate outcomes to more distant outcomes, describes measures, data collection system, and implications for supporting and enhancing project implementation. Opportunities and challenges of evaluating a school and community initiative such as the Kansas LEAN School Health Project are discussed. Finally, the paper concludes with future directions.
