Promoting cardiovascular health for all: A performance-based framework

Cardiovascular disease (CVD) is the leading cause of death in the United States. Each year about 950,000 people die of CVD, many prematurely. CVD death rates and risk factor prevalence continue to be disproportionately high in minority and low-income populations. To address the CVD problem, the National Heart, Lung, and Blood Institute (NHLBI) has set an ambitious goal and rallying theme of "Cardiovascular Health for All" that will drive its programmatic efforts in the 21st century. "Cardiovascular Health for All" is responsive to the Department of Health and Human Services' (DHHS) Healthy People 2010 objectives and the President's Race Initiative, both of which focus on eliminating racial, ethnic, gender, and geographic health disparities. "Cardiovascular Health for All" also addresses issues associated with the underutilization of existing science-based information, particularly in high-risk communities. The purpose of this presentation is to describe the tenets of the overarching "Cardiovascular Health for All" framework. Moreover, the action steps that are being taken by the NHLBI to create and support enhanced dissemination and utilization centers in high-risk communities will be described as well as a new web-based information management infrastructure that will enable NHLBI to offer cutting edge information technology applications, and faster more efficient service delivery of educatiuonal products and services. Included in this infrastruture is a Web-based NHLBI Healthy People 2010 Gateway developed to assist public health professionals in planning, developing, evaluating and sharing results of community-based activities.

Learning Objectives: At the conclusion of this presentation, participants will be able to: 1. Describe the cardiovascular health (CV) framework and performance goals and objectives. 2. State the reasons why there is a need to increase the dissemination and utilization of CV health information. 3. Describe the steps that NHLBI took to create new community-based partnerships and networks to promote cardiovascular health in high-risk communities
