This presentation will describe the concepts and experience from partnerships across the United States that underlie the VALUES model for governing and managing community partnerships. The VALUES model represents Vision, Alignment of resources, Linkage of critical stakeholders, Utilization Improvement, Evaluation, and Synergies.
The VALUES model stresses: (1) the coordination of the roles and activities public health and health care organizations; (2) the composition of the partnership emerge from the alignment of organizational visions and missions with common, related health and non-health outcomes; (3) the criticality of partnership management rather than primarily its governance; (4) the achievement of operational coordination and integration as near-term results of program efforts; (5) the use of explicit processes based on CQI principles to achieve program results and outcomes; (6) the maximization of formal linkages through the creation of partnership policies that guide institutional partner policies and actions; (7) the importance of achieving beneficial changes in service utilization within the partners, the partnership, and the community; (8) the creation of a pragmatic evaluation that produces managerially useful information for partners and the partnership; (9) the achievement of locally agreed to health status and health system improvements; and (10) the creation of explicit, enumerated synergies creating both collaborative and competitive advantages for partners and the partnership.
The critical focus of these efforts is to achieve synergies that reflect system and institutional benefits and identify system savings that reward participation and sustain both operations and innovation.

Understand the VALUES Model for improving governance and management of community health partnerships. 2. Identify critical success and warning factors to success at each stage. 3. Coordinate VALUES efforts with state and national initiatives in public health performance improvement and Healthy People 2010
