Medical Errors at the VA and Beyond

The Department of Veterans Affairs has published a report detailing medical errors it found in its own facilities. This session will consist of a description of the agency's methods and findings, a discussion of how the VA is working to reduce errors, and a summary of the Clinton Administrations's effort to eliminate as much as possible the error identified in the broader health care industry by the Institute of Medicine in its report, To Err is Human. . .
