The Evaluation Tool Kit for Community Health Worker (CHW) Programs aims to make program evaluation easier and more effective for many CHW programs by providing a guide to appropriate evaluation strategies and methods, a directory of evaluation tools and instruments as well as examples of specific tools, and a resource directory of evaluators with experience in CHW programs. The Tool Kit has been developed at the University of Arizona Rural Health Office with support form the Annie E. Casey Foundation. It was created in response to the findings of the National Community Health Advisor Study, also funded by the Annie E. Casey Foundation. The study concluded that published CHA/CHW evaluations were few in number and often limited methodologically. Yet, evaluation was seen as a major component in a program's ability to sustain itself beyond its initial funding period. The Tool Kit is one attempt to help CHW programs succeed. In addition to assembling currently available tools, such as health status reports, quality of life indexes and self-efficacy scales, the Tool Kit has commissioned new tool development specifically for CHW programs, including a client satisfaction instrument and a cost-benefit analysis tool. Several case studies of successful programs at the national, state and local level will also be presented to illustrate various approaches to evaluation. Several possible dissemination strategies will be presented and discussed, including hard copies, CD-ROM, an interactive web site, evaluation training for CHW programs, and ongoing technical assistance.
