Breast cancer incidence and mortality disparity in Missouri

OBJECTIVES: This study examined the time trends for breast cancer among White and African-American women in Missouri between 1980 and 1996. METHODS: Our analyses were based on data from the Missouri Cancer Registry and the Center for Health Statistics, maintained by the Missouri Department of Health. Incidence and mortality rates were standardized to age, and the 1970 US standard. The estimated annual percent change was calculated using log-linear analysis. RESULTS: The age-adjusted incidence rate was 94.7 per 100,000 for Whites and 82.6 per 100,000 for African Americans from 1985 to 1996. Invasive breast cancer incidence decreased 0.2 % annually among Whites and increased 0.8% annually among African American. The in situ breast cancer incidence increased significantly 9.4% annually among Whites and 9.8% among African Americans. Between 1980 and 1996, the age-adjusted mortality rate was 25.1 per 100,000 for Whites and 32.9 per 100,000 for African Americans. Breast cancer mortality increased 0.8 % annually among Whites and 1.3% among African Americans from 1980 to 1990. However, breast cancer mortality decreased 2% annually among Whites and increased 0.2% annually among African Americans from 1990 to 1996. CONCLUSIONS: Breast cancer incidence and mortality have been decreasing in the 90’s among Whites but not among African Americans. The significant increased in in situ incidence can be attributed to screening programs undertaken to detect this neoplasm. Continued surveillance of breast cancer coupled with etiologic research may determine differences in the distribution of risk factors for breast cancer between Whites and African Americans in Missouri.
