Abstract Text: Objective: Participants will gain insights into the synergistic effect of community collaboration that followed a county health survey where asthma was identified as a public health priority. Participants will be shown a tool that will enable them to address disparities in care, and measure interventions and outcomes.

Abstract Text: In 1995 a countywide health assessment identified asthma as a community health needs priority. An asthma consortium comprised of Public Health Nurses, other health professionals, and community health agencies initiated a program among inner city middle school students diagnosed with moderate to severe asthma. Data was collected from pulmonary studies, allergy testing, environmental assessment, and asthma education at school and in the home. Pre and post evaluation of the program revealed an increased asthma knowledge base resulting in a decline in absenteeism and exacerbations. To standardize asthma education and measure outcomes, a comprehensive tool was developed to address this community health need.

Conclusion 2000: Disparities in asthma management exist in the broader population. Therefore, the holistic asthma management tool is now utilized on all ages of asthma patients.

Learning Objectives: Participants will gain insights into the synergistic effect of community collaboration that followed a county health assessment survey where asthma was identified as a major public health priority. In addition, they will be shown a tool that will enable the health care professional to address disparities in care and measure interventions and outcomes
