WomenCare Plus:A study of barriers to ovarian and other gynecological cancer screening in large women

Large women are considered to be at increased risk for ovarian and other cancers. Prior research shows that as weight increases women are more likely to delay or avoid pelvic exams and "Pap" smears.The goals of this two year study funded by the California Department of Health Cancer Research Program are 1)to identify specific barriers to ovarian and other gynecological cancer screening and preventive care for large women, 2)to determine if women delay seeking care because of these barriers, and 3)to make specific recommendations regarding improving access to care. Findings are reported from focus group and interviews with 60 African-American and White urban and rural large women, aged 40-60 and 30 health care practitioners. Principal findings include: 1) women, regardless of size, educational level, or race are unaware of symptoms, early diagnosis, and treatment of ovarian cancer, 2) large size significantly impacts women's utilization of preventive care secondary to stigmitizing cultural attitudes,medical practices,and practitioner-patient interactions,and 3)women have developed specific strategies that mediate the negative impact of these barriers. Recommendations include practitioner training and consumer education focused on positive attitudes, mutual respect, clinical skills, patient advocacy, and appropriate medical settings and equipment. Improved quality and access to gynecological cancer screening and care for large women will offer significant benefit to all women.

Learning Objectives: At the conclusion of the session, the participant in this session will be able to: 1. Identify at least one strategy large women have employed to reduce the negative impact of barriers to care. 3. Identify at least one recommendation to improve access and quality of preventive care for ovarian and gynecolocal cancer for large women
