Undergraduate students working in a community assessment nursing practicum focused on promoting optimal wellness in clients residing in a HIV/AIDS residential facility in an urban city in the southwest. This poster describes the process that students followed to address health needs in this population, the outcomes of the interventions, and the effectiveness of student initiated interventions. The structure and format for the student learning experience, strategies that enhanced student learning to 'partner' with the community, and recommendations for others using this model will be summarized. Working in groups of 5, students completed a "community assessment", which included identifying the target community and their concerns and developing community diagnoses, expected outcomes, major obstacles, interventions and evaluation strategies. Interventions were developed in liaison with residents and community leaders, and the need for a comprehensive outcome (versus process) evaluation was identified. Assessment of residential program effectiveness was completed and included residents' perception of the usefulness of program resources. This student-led evaluation was used by the agency in documenting effectiveness to both public and private funding audiences. Results of this assessment prompted a student- initiated program to address residents' social isolation. Students gained experience and understanding of the complex and diverse needs of this target group and the multidisciplinary roles of community health professionals within the urban setting.

Learning Objectives: 1. Describe selected student-initiated interventions to improve health outcomes in a residential HIV/AIDS facility. 2. Identify process, outcomes, and effectiveness of student-led interventions 3. Describe outcomes for the population at the HIV/AIDS residential facility. 4. Discuss recommendations for other multidisciplinary interventions for this target population
