Substance abuse during pregnancy is a serious problem with significant adverse effects on women and their babies. This session will describe the development, implementation, and evaluation of the Early Start Program, an innovative, obstetric clinic-based, perinatal substance abuse treatment program in a managed care organization (MCO). Early Start began as a pilot program in 1990 and has expanded to include 16 clinic locations. Treatment is provided in the obstetrics clinics by Early Start Specialists, licensed therapists with expertise in substance abuse. Reports providing feedback to clinicians have been developed, including biannual reports summarizing universal toxicology screening results and weekly reports for the Early Start Specialists identifying women with positive toxicology tests. For the evaluation, a cohort of 33,000 women was defined who delivered babies or had fetal losses from 7/1/95 through 6/30/98 and who completed an Early Start prenatal screening questionnaire to assess their substance use before and since pregnancy. The evaluation process required the linkage of numerous electronic databases to form a comprehensive database containing Early Start questionnaires, patient assessment, and treatment data; toxicology screen results; and neonatal outcomes, which we have the unique ability to capture electronically. Treated and untreated substance abusing pregnant women were compared to controls who did not abuse any substances on rates of neonatal outcomes, such as Neonatal Intensive Care Unit admission, assisted ventilation, low birthweight, and preterm birth.
