This session will present results from a community-planning process undertaken by the East Side Village Health Worker Partnership on Detroir's east side, a project of the Detroit Community-Academic Urban Research Center. The Partnership began in 1996, and has as its main goal to address underlying social determinants of health on Detroit's east side. Partnership membeers include the Butzel Family Center, Detroit Health Department, Kettering-Butzel Health Initiative, Friends of Parkside, Henry Ford Health System, Warren Conner Development Coalition, Islandview Development Corporation, VISIONS, the East Side Parish Network, and approximately 30 community members who are active as Village Health Workers or lay health advisors. The session will describe a community planning process undertaken by the ESVHWP, and the process used to bring together multiple perspectives and information sources to arrive at Partnership priotities. Several of the specific projects that have been, or are being developed as a result of this proces will be discussed, along with challenges and facilitating factors in the development of those efforts.
