Integrative medicine in comprehensive AIDS programs

The findings of an empirical study that explored the organizational approaches of three AIDS adult day health care programs with integrating complementary/alternative and conventional therapies will be presented. The study used a grounded theory research approach to characterize the programs' experiences and the use and perceived benefits of integrative therapies by providers and clients in the program. These findings were used to develop a theoretical framework for conceptualizing the key components and processes of integration that enhanced the well being of both the individual and the program community, issues that have been largely missing from studies investigating the use and efficacy of complementary and alternative medicine. The core category that emerged from the data analysis was "finding common ground," or the process of developing a common set of beliefs, values, language, and goals around the disease (and other client needs) and its treatment. Common ground is the hallmark of an integrated program and may be enhanced by a program structure that promotes communication and connection, two essential processes of integration. The report has important implications for research and practice. Specifically, the framework and findings suggest that in order to account for the experiences and outcomes associated with integrative therapies, researchers should consider the social context within which these services are delivered and use measures that take into account not only the individual but also the therapeutic relationship and systems issues. Similarly, the report suggests clinical approaches for persons with AIDS should address complex needs holistically.
