The history, mission, program of care and outcomes of a community-driven academic nursing center, Temple Health Connection (THC)related to teen pregnancy will be shared. In the census tracts served by the academic nursing center, city data from 1995-1998 indicates the births to mothers under 18 years of age decreased by 17.1% versus a citywide decrease of 9.4%. The live births with insufficient prenatal care dropped by 31.3% versus a citywide reduction of 7.8%. The number of low birth weight babies increased in the census tracts served by THC, but of the 24 births to mothers followed at THC during this time frame, none were low birth weight. The presentation will include factors that the staff feel could be replicated.
