Maximizing health outcomes for vulnerable populations: Lessons from nurse-managed health centers

This paper will focus on best practices of member centers of the National Nursing Centers Consortium (NNCC, an umbrella organization for 36 nurse-managed health centers. It will describe the work of selected centers and the NNCC in primary care and public health, presenting several specific program strategies and an overview of the impact and outcomes of services, education, and research achieved in these community-driven practice models.
