Responsibilty Centered Case Management as An Instrument to Improve Access to Population Based Health

Introduction: Access to public health services among underserved populations is an essential aim of Public Health Nursing in Fulton County, Georgia. Approximately 13% of Fulton County adults reported needing to see a doctor in the last twelve months but were unable to do so due to cost. Recent estimates state that 14.4% of Fulton County residents are uninsured and 3.5% utilize medicaid. The purpose of this study was to evaluate the contribution of Responsibility Centered Case Management on access to healthcare for Maternal and Child Health services. Methodology: Teams consisting of Public Nurses and Community Health Outreach Workers, each with defined roles, were established to function as highly cohesive care units at 15 public health centers. Integrated supervision and accountability of all duties for the team were delegated to the Nursing Supervisor of the health center rather than program specific supervision. Results include establishment of optimal case load, increased enrollment of clients for early prenatal care, age appropriate immunizations, evaluation of the physical, developmental/nutritional status of infants, comprehensive psycho-social support and increased enrollment of clients in parenting education classes.
