What would Dr. Donabedian think about 'medical error'?

Former Chair of the QIC Dr. Gordon Schiff will open the session with a presentation spotlighting the contemporary relevance of a particular work of Dr. Donabedian. This year the talk will draw upon a variety of Dr. Donabedian's writing to examine their relevance to a theme related to that of the work of the awardee (Judy Smetzer and the Institute for Safe Medical Practices) -- diagnostic errors. Drawing on the work of the Cook County Hospital-Rush Developmental Center for Research Patient Safety (AHRQ funded project # # HS 11552) cross-linking themes and useful Donabedian insights will be explored. These include definitional issues in teasing out errors from near-misses and operations failures using Donabedian's structure-process-outcome triad, Donabedian's "trajectory" study design, his ideas about "sleuthing" for abnormal lab tests, as well as visions for rededicating passion and commitment for practice based learning and improvement. The presentation addresses the question: What would Dr. Donabedian think and say about medical error?
