The American Academy of Pediatrics, the American Academy of Family Physicians, and the national Maternal and Child Health Bureau are promoting the slogan “Every Child Deserves a Medical Home.” What is a Medical Home? How do clinicians, public health practitioners, and health plans put Medical Home into action for children with special health care needs?
One way to implement Medical Home is to develop specialized health education materials for parents that:
* Help organize health records and information in order to optimize contact with health providers and health plans;
* Provide resources and specialized information about caring for a child with special health care needs; and
* Improve communications among families, health providers, and health plans.
This session will discuss the process of developing a resource manual for parents of children with special health needs. The presenter will propose an approach to building collaboration with health insurance plans, health care providers, family support agencies, and other State and non-profit agencies to support the resource manual. Highlights from the evaluation of the Massachusetts resource manual, Directions: Resources for Your Child’s Care, will be shared.
