Healthy People 2010 calls for increasing the proportion of States that have service systems for children with special health care needs (CSHCN); and the federal Maternal and Child Health Bureau is supporting development of such systems through the Title V MCH Block Grant Program and discretionary grants. MCHB efforts focus on improving access to a medical home; meaningful family participation in medical decision making and policy development; making systems easy to use; increasing availability of public and private health insurance, and facilitating transition of youth to adulthood. This session focuses on the strategies and mechanisms for developing, sustaining, and evaluating the medical home; and for supporting families in their critical roles as care-giver and systems change agent. Specific issues to be addressed include key components of the medical home; impact on families of caring for CSHCN; barriers faces by CSHCN and their families in underserved areas; community-based perspective on collaborative development medical homes; role of medical homes within the health care safety net; role of providers in partnering with and supporting families with CSHCN; and methods for monitoring and measuring the impact of the medical home on CSHCN and their families. | |||
