Taking the initiative: Implementing the American Heart Association Guide for improving cardiovascular health at the community level

Background: U.S. health objectives, as presented in Healthy People 2010, have placed a national emphasis on preventing heart disease and stroke and its risk factors. This report describes a model and strategies needed to translate the American Heart Association (AHA) Guide for Improving Cardiovascular Health at the Community Level into action to prevent and eliminate heart disease and stroke disparities. Methods: Members of The Healthy People 2010 Heart Disease and Stroke Partnership conducted a literature review and identified resources to develop a framework for implementing the AHA Community Level Guide. Representatives from CDC's State Heart Disease and Stroke Prevention Program and from Native American community groups reviewed the framework and provided feedback to make it a user-friendly document for communities. Results: The Taking the Initiative Framework includes the following steps: assessment, community-based planning, and widespread and sustained implementation. Every step in this cycle is supported by community mobilization and evaluation. Community mobilization involves identifying assets, resources, leadership, community capacity, and developing partnerships, trust, and a shared understanding of community participation and control that is supported by all the stakeholders. The Taking the Initiative Framework also includes a flowchart based on a socio-ecological logic model, assessment and evaluation measures, and resources to assist in selecting and implementing the AHA Community Level Guide. Conclusions: A comprehensive approach that addresses the environmental, social, and cultural aspects of health, as well as individual lifestyles and behaviors, offers the greatest prospect for achieving cardiovascular health of populations.
