Florida Chronic Disease Health Promotion and Education Program

The Florida Department of Health has created a unique statewide model for chronic disease prevention in every county in Florida. The Chronic Disease Health Promotion and Education Program coordinates funding to all 67 Florida County Health Departments to implement policy, environmental, and educational interventions around five Healthy People 2010 objective areas: heart attack/stroke, diabetes, nutrition/overweight, physical activity, and tobacco. These programs mobilize community partnerships to target communities, schools, worksites, and other groups to implement policy and environmental interventions that will have a lasting impact on health behaviors. Funding is provided through a combination of state funds, the Preventive Health and Health Services Block Grant, the CDC Heart Disease and Stroke Prevention Grant, and other categorical grants.

County coordinators submit an annual work plan that includes: the Healthy People 2010 objectives selected; local impact objectives; policy, environmental, and education interventions, activities, and indicators; community partners; media activities; and an evaluation plan. Coordinators are provided with a programmatic guide that includes an inventory of all the policy, environmental, and educational interventions that have been submitted over the two years the program has existed.

Program evaluation includes success stories of completed interventions that have been evaluated and a summative inventory of all policy and environmental interventions implemented across the state. A state document entitled “Promising Practices” facilitates the replication of successful interventions in other counties. To date, over 300 policy and environmental interventions addressing the five risk factors have been proposed statewide.
