Improving access to care for rural residents through the Brazos Valley Health Partnership, a Healthy Communities Access Program

In 2002, the Center for Community Health Development in conjunction with several community partners, conducted a comprehensive health status assessment of the Brazos Valley. The assessment found that a large disparity existed in healthcare between the residents of the six rural counties of the region and the residents of the main urban Brazos County. An outgrowth of the assessment was the creation of the Brazos Valley Health Partnership (BVHP), a regional partnership of various community organizations and individuals from all sectors of the community. The partnership worked to address fragmentation of the health care delivery system by developing health resource centers in the rural counties of the Brazos Valley. Through these centers, the BVHP hopes to improve the capacity of the rural communities by leveraging existing resources and facilitating creative partnerships among service providers.

In September of 2003, the BVHP was awarded a Healthy Communities Access Program grant by the Health Resources Services Administration. The major activities of the partnership are to integrate and coordinate services through co-location and centralized case management; to provide medication assistance; and to develop a transportation system for low-income residents. Findings from evaluation of this project could lead to significant local and state policy changes in the improvement of access to healthcare for uninsured and underinsured residents. This presentation will provide an overview of the project, its goals, objectives and current status and serve as background for the subsequent panelists.
