Using Evaluation Data for Planning and Policy Development: A Case Study in the Brazos Valley, Texas
In 2003, the Center for Community Health Development and the Brazos Valley received funding from HRSA as a Healthy Communities Access Program (HCAP) Community. This three-year project focuses on improving access to care and health status for rural residents of the Brazos Valley through coordination, increased efficiency and increased effectiveness of healthcare through the development of local health resource centers and community partnerships. We propose a panel presentation focusing on: (1) an overview of the project and evaluation plan, (2) data collection methods and preliminary data from the data sets comprising the evaluation, and (3) use of the data for planning and policy development. We will focus on strategic and operational planning within a community-based partnership, as well as policy development for local and state agencies to facilitate partnership-driven health improvement strategies. The partnership currently has resource centers in three of the six rural counties of the Brazos Valley region and will be expanding to each of the remaining counties in the next few years. The recent designation of the Center for Community Health Development as a Prevention Research Center by the Centers for Disease Control and Prevention has enabled the project to look at the feasibility of extending the concept to other contexts to test its generalizability in areas in which the population experiences similar challenges to obtaining access to care.
Learning Objectives: By the end of the presentation, participants will be able to: 1. Describe methods and measures for various aspects of a partnership-based strategy for health improvement. 2. Identify ways to interpret and report data to multiple audiences for planning and policy development (i.e. what type of data is relevant to different audiences) 3. Apply lessons from this case study to similar community-based strategies.
