The Brazos Valley Health Partnership, a national Healthy Communities Access Program of the Health Resources Services Administration, is working to decrease fragmentation of the health care delivery system by improving access to care for uninsured and underinsured residents of the six rural counties of the Brazos Valley. Included in the evaluation of this project is the measurement of network linkages between participating organizations. An array of methods was used to monitor changes in relationships among and between organizations participating in the project. Additionally, community leaders were interviewed and surveyed to assess any changing perceptions of the issue of access to care. Interviews, observations of meetings and survey instruments were used to document baseline and trend data for the first two years. Results from these various assessment approaches will be presented. Further, the presentation will explain how to use the data to help in the planning of subsequent project activities.
