The Centers for Disease Control and Prevention (CDC), Division of Oral Health has developed a reference tool for creating state Burden of Oral Disease documents. This can be a vital component of state oral health program surveillance systems. The tool is an application of the chronic disease model for describing disparities in health as the burden of disease on individuals, disparate groups and society. The rationale for this approach and elements of the tool and their use will be reviewed. The tool focuses on the indicators contained in the National Oral Health Surveillance System. Targeting specific audiences with compelling information drawn from a burden document will be emphasized. An extensive appendix will be described that includes resources, sample burden documents, sample fact sheets and policy briefs as well as a calendar of expected data release dates from national surveys. References will be given for accessing the compact disc and website versions for creating state burden of oral disease documents.
