Body Mass Index (BMI) and Waist Circumference (WC) as indicators of risk for type 2 diabetes (T2DM) and cardiovascular disease (CVD) in Hispanic children

Hispanic children (132) at risk for T2DM and their siblings, were examined for T2DM and CVD risk factors based on physical examination, family history, and laboratory studies. Children were divided in four categories for BMI and WC (<75th, 75th-84.9th, 85th-94.9th, and >95thpercentiles) to assess the hypotheses: elevated indicators of risk would be present at the 75thpercentiles of BMI/WC; WC would be more helpful in identifying elevated risk when contrasted with BMI.

Systolic blood pressure was consistently and significantly increased as the BMI/WC increased form 75th-95th percentiles, as well as the acanthosis nigricans (AN) prevalence. At WC 75thpercentile, AN was more than twice than of the prevalence at BMI 75thpercentile. Fasting glucose was normal (<100mg/dl) for most children, except for 12 children (100-112 mg/dl). In contrast, mean insulin levels were elevated (°Ý15uU/mL) starting at 75thpercentile for both BMI/WC, while insulin resistance, determined by homeostasis assessment model for insulin resistance (HOMA-IR), was also elevated starting at the 75thpercentile of BMI/WC. Higher HOMA-IR mean levels were detected in the WC>75thpercentile compared to BMI (4.7, 5.4, and 7.6mmol/L and 4.1, 5.2 and 6.8mmol/L respectively). Triglycerides, total cholesterol, and LDL-C mean values increased consistently and significantly at BMI/WC>75thpercentiles and were over the mean and/or at the 95thpercentiles for Hispanic children in the third National Health and Nutrition Examination Survey (NHANES III. The findings indicated that elevated risk for T2DM and CVD were already present at 75thpercentiles of BMI and WC. In this group of children WC is a better predictor of risk than BMI.
