Community Health Workers (CHW) have very important roles in the ongoing operation of the Health Care Delivery Team. These roles vary significantly, depending on whether the role is developed as a part of an intervention or as a support system for patients. This session will explore various ways that the CHW have been utilized and the effects of their role in delivering a continuum of care as a part of a Health Care Delivery Team. | |||
Learning Objectives: At the end of this session, the participants will be able to: 1. Assess different organizational environments that employ CHWs as a part of the Health Care Delivery team. 2. Articulate the roles and responsiblity of the CHW supporting diabetes self-management with Latino populations. 3. Recognize the benefits and challenges of the efforts of CHWs working in remote and rural locations. | |||
