An innvestigation and assessment of health management pattern base on community healing centre

[Abstract] Objective: To study the a Health management patter base on community healing centre and evaluate its practical effect on community crowd. Method: the datas of research subject are collected from Lianhuabei community health centre in Futian district. Population and disease distribution character was documented and health education, healthcare record buiding, home healthcare with community outpatient service, and regularly follow up were performed in order to operate health management such as three class prevention to different kind of diseases. The effect of community disease control are evaluated. Result: After one year's implementing of the health management program,the awareness of health protetion in the whole community of Lianhuabei is evidently increased and the cognition rate of unhealth food habit and unhealth life style has been greatly improved. The degree of self-health satisfaction and quality of life has been obviously improved(P<0.05).Conclusion: An individualized health management pattern with good plan and organization based on community health centre is good for implementing our “everyone can enjoy primary healthcare” policy, saving health service resource of the country and effectively enhancing the quality of life and health level of the community. Such kind of health management program is worth of popularizztion throughout the country. [Keywords] Community healing center, Chronic Non-Communicable Diseases, Health Management pattern .
