Roundtable
This session addresses various health and health care issues among the older adults. Topics include but are not limited to: giving voice to older patients in a child-rich Medicaid health plan, improving care transitions for older adults, effective community-based care management model for chronically ill older adults, and unmet needs and delayed healthcare.
Session Objectives: 1. Contrast differences between access barriers reported by older patients and other patients to identify areas for improvement in services. 2. Identify common transitional care issues encountered by older adults 3. Describe an effective model of community-based care management for chronically ill older adults
