The Patient Centered Care Collaboration to Improve Minority Health (PCCC) Initiative considers stakeholder engagement a critical component of efforts to promote the adoption of patient centered outcomes research (PCOR)evidence. Stakeholder engagement occurs at multiple levels—the national, local and individual provider and consumer levels.This session describes the use of a model framework that is used to support stakeholder engagement project-wide and highlights how tiered partnerships are used to implement dissemination and adoption efforts. The PCCC involves stakeholders of various types(i.e.,patients, providers, professional associations, community-based organizations, and health systems organizations) who work in partnership across a continuum of activities supporting PCCC efforts. This presentation will highlight the pivotal roles stakeholders play in helping to influence the uptake and adoption of PCOR evidence, hasten the speed at which the adoption process occurs, and support the acceptance of interventions that support medication adherence, diabetes and hypertension self-management, as well as lifestyle choices in support of weight management. PCCC employs a broad range of strategies that aim to heighten interest and demand for receipt of key interventions, information, materials, and resources.In addition to highlighting national stakeholder engagement strategies, the session will also highlight experiences of two geographically diverse partner programs— The Chicago Hub and the Houston Hub. The Chicago Hub's HELP (Health Empowerment Lifestyle Program)is based at the University of Illinois-Chicago.In Chicago, the HELP program involves a collaborative partnership with a community health center, the Lawndale Christian Health Center, where community health workers are instrumental in facilitating group health education classes focusing on lifestyle modifications.
