3.Describe how the Chicago Health Empowerment Lifestyle Program (HELP) can be used to promote self- management of hypertension, diabetes and healthy weight.
4.Describe how the Patient Centered Medical Home model can be used to manage medication use.
5.Describe how pharmacists can be used with health educators to have an impact on behavioral outcomes.
