The presenter will explain how the Joint Center has helped our two affiliates from Baltimore, MD and Bernalillo County, NM increase access to and use of internet-based tools to reduce inequities in cardiovascular disease, infant mortality, and diabetes by improving neighborhood conditions.

I have responsibility for HPI’s “PLACE MATTERS” a key initiative to improve the health of participating communities. Nineteen teams are working in 27 jurisdictions across the country to address conditions that lead to poor health.
