CDC Racial and Ethnic Approaches to Community Health (REACH) Minority Serving Organization-Dissemination

The Centers for Disease Control and Prevention (CDC)-funded program addresses via the National Organizations that Serve Minority Communities Racial and Ethnic Approaches to Eliminate Health Disparities (MNOs REACH-US), strategies that eliminate racial and ethnic differences in health beyond a focus on a particular disease, as well as the broader context of social determinants of health. MNOs with a national reach and broad community development agenda are uniquely suited to integrate strategic actions for community empowerment within a community health program. In this session grantees will discuss strategies that eliminate racial and ethnic differences that are being disseminated at the local level with Racial and Ethnic Approaches to Community Health (REACH) funding and lessons learned that can be transferred to other communities.
Session Objectives: Identify the five A’s in the Five A Model (behavior change model). Explain the difference between traditional and commercial tobacco. Discuss lessons learned and strategies that eliminate racial and ethnic differences that are being disseminated at the local level with Racial and Ethnic Approaches to Community Health (REACH) funding.
