Advancing Health Where it Starts: Community Practice in Minnesota

This session will focus on the concept that health happens where we live, learn, work, and play. Considering these multiple dimensions that create health, health departments and their community partners are working on new ways to address the causes and consequences of disease, form new partnerships to help people live healthier lives, and make policy and environmental improvements that enable people to make healthier choices. This session will focus on partnerships, programs and activities that are ongoing in Minnesota and made Minneapolis and the state leaders in advancing health.
Session Objectives: Name at least two innovations in community practice Describe how these innovations could be adapted/adopted in their own jurisdiction
