Background. Improving community health requires primary care medical providers partnering with public health officials and community stakeholders. 
Purpose. To describe a partnership between Montefiore Medical Center (MMC), the Bronx District Public Health Office of the New York City Department of Health and Mental Hygiene (NYCDOHMH), community-based organizations, and political leaders to improve the health of patients and communities surrounding 6 MMC-affiliated health centers.
Methods. Bronx-CATCH (Collective Action to Transform Community Health) began in 2010 as a multi-stakeholder partnership to implement and evaluate coordinated health-promoting efforts, including: (1) geospatial mapping of the health centers’ neighborhood resources; (2) free Zumba classes, Playstreets (car-free streets where residents can engage in physical activity), health screenings, and retail food store engagement to promote healthier food options; (3) patient-based services, including referrals to the YMCA Diabetes Prevention Program (YDPP) and ‘prescriptions’ that can be used to buy fresh produce at collaborating stores; and (5) systematically incorporating questions on diet and physical activity, from the NYCDOHMH-administered annual Community Health Survey (CHS), into routine patient assessments documented in the electronic medical record (EMR) of the 6 MMC clinics.
Results. Activities in 2013 included 27 Zumba classes (n=152), 5 health screenings (n=278); 5 Playstreet sessions (n=536), 9 retail food store engagements, 13,000+ patients with CHS question results in the EMR, 6 health centers providing YDPP referrals and 2 providing produce prescriptions.
Conclusion. A multi-stakeholder collaboration to promote community health, with integrated evaluation methods tracking clinical and population health outcomes, shows promise to improve the health of patients and their communities.
