Using Telehealth and HIT to Improve Health Outcomes in Rural Community Health Centers

Finger Lakes Community Health, a FQHC in Upstate New York, has pioneered the use of  telehealth programs to improve care to underserved populations in their patient centered medical home. A model of care that utilizes EMR registries, care managers, community health workers, telemedicine, teledentistry, and telehealth has significantly improved outcomes for patients with diabetes, early childhood caries, HIV, Chronic Hepatitis C, children with ADHD, and for adults with mental health disorders. Outcomes have demonstrated timely access to preventive screening, treatment, and specialty care, improved quality/performance measures, decreased symptoms, and improved function for patients. This presentation will give an overview of the current state of telehealth and program development details for implementing telehealth services. The use of HIT and telehealth to facilitate successful health outcomes for patient seen in rural primary care will be demonstrated.
