This study aimed to evaluate the racial disparity in influenza vaccination among working-age diabetes patients over years 2005 to 2011 in the U.S.

Diabetes patients aged 18-64 years were extracted from Diabetes Care Survey of 2005-2011 Medical Expenditure Panel Survey. Influenza vaccination status was defined by having flu shot or not in each year. Influenza vaccination rates were compared between groups: non-Hispanic whites (NHW) and minorities. Multivariate regression models were used to control for age, gender, marital status, education, private/public insurance, family economic status and residential regions.

The overall influenza vaccination rates increased from 45.82% in 2005 to 56.47% in 2011. Despite the improvement in both groups, the NHW group had higher rates for all the years, with the differences varying from 5.12% in 2011 to 13.42% in 2005 (statistically significant in years 2005 to 2009). Controlling for influencing factors, the adjusted odds ratio of race/ethnicity were still significant in 2005, 2006 and 2009. Older age and public/private insurance coverage (compared to no insurance) were significant factors for all years consistently.

The racial disparity in influenza vaccination persisted among working-age diabetes population. Expanding insurance coverage for minorities may improve influenza prevention when the Affordable Care Act is implemented.
