Interprofessional care involves developing an infrastructure and mutual philosophy of care to facilitate communication among diverse health professionals in the care of patients and populations. Health care reform is taking on many faces to improve the overall health of patients, encourage collaboration among providers and community resources, and reduce the cost of care.  Future providers must be prepared for new roles and models of care in order to make this a reality.  The aim of this project is to develop an expectation of collaborative care among our future health care providers such that they will be prepared for their role in health care delivery.

The presentation will describe the process used by an academic health center to encourage the interaction among health professions students in order to develop an Interprofessional Care Access Network (I-CAN) for a disadvantaged population.   It will specifically focus on the integration of dental students into a network that includes nursing, pharmacy, dental and medical students.  The network also includes community agencies, health clinics, and providers involved in the care of the target population. The presentation will describe the process used for development and implementation of the I-CAN in collaboration with those community partners.   The successes, barriers, and student feedback will be highlighted to assist our public health colleagues in their own development of an interdisciplinary care access network.
