How can health care and public health work in partnership, with defined roles and responsibilities, to impact the health of a community?  During this interactive session for APHA Annual 2014, a framework developed and tested for successful collaboration will be presented and discussed.  Participants also will have an opportunity to explore driving and restraining forces to health care and public health collaboration, and ways to prevent failure once collaboration has been established.

The framework for successful collaboration to be discussed focused on engaging the health care and public health communities to address appropriate use of antibiotics in three jurisdictions. Overuse and misuse of antibiotics, and prescribing broad-spectrum drugs when they are not needed, are causing a wide range of problems globally. A recent report from the Centers for Disease Control and Prevention notes that overuse of antibiotics is attributing to antibiotic resistant bacteria, making antibiotics far less effective worldwide.  A Public Health Antibiotic Stewardship Driver Diagram was developed and tested to help promote alignment of health care and public health efforts to increase appropriate antibiotic use.  Successes and success factors of this initiative will be discussed during this session.

Participants of this session also will have the opportunity to work in small groups using three quality improvement (QI) tools to explore: 1) some of the driving and restraining forces to health care and public health collaboration on an important community health issue; 2) how to prevent failure once health care and public health collaboration is established using a Process Decision Program Chart; and 3) how to use a Driver Diagram to map out an intervention with a public health or health care partner.

The framework presented during this session can be utilized on any health issue where collaboration between health care and public health can result in improved community health.
