Health Impact Investments: Innovative payment models to support clinical and community partnerships and self-management for the aging population
The result of the work in the past year has been the identification of key elements of model communities that would support further health systems improvement and consideration of new payment models including social impact bonds and health impact investments. The presentation will include an outline of the components of a model community and which components are foundational for next steps in bringing in innovative financing models. It will include a summary of existing finance models and ones being considered for the next step in SMA Model Communities initiative.
