The Self-Management Alliance (SMA) is a tri-sector partnership created to promote self-management in older adults.  A key initiative of the SMA is the Model Communities initiative.  This project seeks to identify communities with a history of strong health systems partnerships to improve linkages between the health system and community-based organizations to support self-management in aging adults. Identified communities were vetted for participation in innovative funding models that would help support Patient-Centered Medical Home proposed 2014 Standards that include integration of self-management and work with community organizations.

The result of the work in the past year has been the identification of key elements of model communities that would support further health systems improvement and consideration of new payment models including social impact bonds and health impact investments.  The presentation will include an outline of the components of a model community and which components are foundational for next steps in bringing in innovative financing models.  It will include a summary of existing finance models and ones being considered for the next step in SMA Model Communities initiative.
