Highlighting Community Efforts and Achievements around Healthy People 2020

Healthy People 2020 (HP2020) is the longest running Federal disease prevention and health promotion plan in the United States and establishes science-based measurable objectives with targets to be achieved over a ten-year period.  HP2020 measures progress over the decade by tracking data for more than 1,200 objectives and provides stakeholders with actionable information, evidence-based resources, and best practices to improve the health of communities around the country. In a survey of HP2020 stakeholders, respondents expressed an interest in sharing data and evidence-based information to aid in HP2020 implementation. By highlighting successful and innovative community efforts, HP2020 is building grassroots, community support for HP2020 and aiding in the translation of evidence-based approaches at the community level.

HP2020 highlights community-based best practices in four ways—the online Sharing Library, “Who’s Leading the Leading Health Indicators?” webinars, Progress Review webinars, and online eLearning lessons. The Sharing Library allows health professionals to learn what others across the country are doing to implement HP2020 and to share their own stories of working toward HP2020 objectives. The “Who’s Leading the Leading Health Indicators?” and Progress Review webinar series each showcase states, communities, or organizations that are making progress on various HP2020 objectives. Lastly, our eLearning lessons use a case study approach to feature a community working to achieve HP2020 objectives and are designed to help students and health professionals learn how to reach our national health goals.

Presenters will describe lessons learned in using storytelling to highlight successful community efforts and achievements to communicate best practices in disease prevention and health promotion.
