Rush University Medical Center (RUMC) has accelerated its years of educational and healthcare partnerships with the Malcolm X City College (MXC) and the Medical Home Network (MHN) through a five million dollar funding from BMO Harris Bank to improve the health outcomes, education, and employment of the Westside and Southside communities of Chicago. This innovative project is designed to create and evaluate new models of care, develop and train a workforce from the community that supports the new models of care, and rigorously evaluate each component, with a goal of fostering public-private partnerships to further healthcare system transformation. The four project components include (1) developing and implementing new curriculum and training programs wherein RUMC is working with MXC to develop new programs that address the workforce needs of MHN and other medical home models of coordinated care to manage population health in underserved communities. Continuing education programs are being developed in collaboration with MHN to prepare providers for working in the new healthcare delivery models; (2) developing and piloting new care models by BMO Health Disparities Research and Evaluation Fellows to determine how to improve the effectiveness and efficiency of care that is provided to medically underserved communities; (3) developing and testing innovative ways to improve health outcomes in the community in collaboration with RUMC’s community partners and (4) evaluating every component of this project on an ongoing basis to better understand and address barriers to achieve long-term sustainability and replication.

Identify unique aspects of a successful collaboration between academic medical centers, affordable care organizations, and educational institutions to improve education, employment, and health outcomes of underserved communities. Discuss advantages of involving internal and external stakeholders from the beginning, and key leaders’ involvement in overseeing day-to-day operations. Explain the strength of ongoing evaluation which should be guided by key stakeholders’ ultimate goals for the project.
