The Cincinnati Children’s Hospital in Hamilton County, Ohio has initiated a groundbreaking effort to collect comprehensive data about community conditions and to identify “hot spots” where the incidence of disease and injury is especially high. The data is then used to inform strategies to address those conditions and help prevent injury and illness in the first place. In this session, the director of the hospital’s Community Health Initiative will describe how they use patient intake data to hotspot neighborhoods with high asthma prevalence among children in Hamilton County, link this analysis to the prevalence of poor quality housing in those neighborhoods, and work with community partners to address the housing conditions.  He will also detail similar efforts to identify communities with high rates of infant prematurity and child injuries and the partnerships being developed to address improve population outcomes.  He will then share his vision for the applicability of these methodologies to other health conditions and the future directions of this work.
