This session explores the work on Social Determinants of Health (SDOH) within the Healthy People 2020 initiative. Healthy People 2020 is produced by the Office of Disease Prevention and Health Promotion in the Office of the Assistant Secretary for Health. To date, the SDOH topic area has focused on a five domain, place-based model that pulls together both existing HP2020 objectives and develops additional objectives that have not previously been addressed, such as those related to poverty.

 The SDOH portal project will further develop the SDOH topic area and increase its usefulness in the field. This portal will offer a set of tools and approaches to the HP2020 stakeholder that can be used for state and local planning efforts. Most Public Health practitioners are very familiar with the concepts and goals of SDOH; the challenge is making those concepts actionable , and providing  a bridge between theory and practice. Within the Healthypeople.gov website parameters, ODPHP will be able to include state and local approaches to SDOH implementation that can be adapted or replicated. In addition, SDOH tools from organizations such as RWJF, Pew and Wisconsin Match will be included.
