A unique university and public health institute linkage provides population-based services during off hours at a Federally Qualified Health Center.  The goal is toward further community engagement in the primary care clinic.  Teens, women and men in the community are surveyed regarding their health issues and health interests toward planning health promotion and disease prevention programs. Access is provided for the Health Insurance Market Place, nutrition education, exercise, sexual health, prevention of specific communicable and chronic diseases, knowledge of resources within the community, assistance with further education and preparing for job opportunities.  The initial assessment, program plans, program interventions and evaluation of outcomes will be shared.
