Tobacco Free Futures (TFF) is an integrated health systems tobacco cessation model based on best practices. Alberta Health Services Tobacco Reduction Program uses the model to support tobacco cessation across a variety of health care settings within Alberta Health Services as well as affiliate and contracted service providers.

Tobacco Free Futures is a joint project between Alberta Health Services and the Canadian Cancer Society funded through the Alberta Cancer Prevention Legacy Fund. The fund is focused on activities to reduce cancer risk among Alberta’s population of over 4 million people.

Tobacco Free Futures is transitioning from being a demonstration project to system wide implementation as an integral component of Alberta Health Services Tobacco Reduction Program.

The presentation will review how evaluation results from TFF’s 30 initial sites have been used to improve the engagement process with new sites and to address concerns from sites over the resource/workload required to implement the model.  Process and system changes that have facilitated the implementation and sustainability of the model will be discussed. The processes that are used during the implementation at a site or within a program or service area will be described. Resources used in the implementation of TFF will be described including how healthcare providers can access the various web based resources that have been developed.
