The Centers for Disease Control and Prevention (CDC) funds the National Program to Eliminate Diabetes-Related Disparities in Vulnerable Populations, a five-year project to address complications and health disparities associated with diabetes.  The National Program is comprised of six national organizations working with 18 community-based coalitions to implement evidence-based interventions for managing type 2 diabetes in African American, Latino, Asian-American/Native Hawaiian/Pacific Islander and American Indian communities in the United States.  The diabetes management intervention are also targeted o rural communities.

People with diabetes often do not receive Diabetes Self-Management Education (DSME), despite evidence that DSME is an effective intervention.  This presentation will provide an overview of community-based DSME and the significance of cultural tailoring in increasing participation in DSME programs. Additionally, we will discuss findings and lessons learned from culturally tailored DSME programs in African American communities in Michigan, American Indian tribal communities in Kansas and Michigan and Latino communities in California, New Mexico, and Arizona.
