Hidalgo Medical Services (HMS) is the primary health care provider in two rural frontier counties in New Mexico, seeing over 90% of the population in one of the counties at some point. Given that responsibility for the population and the clear social, economic, and physical environment factors shaping health, HMS has a long history of and commitment to working on both treatment and community prevention. HMS has established a number of initiatives that bridge clinical service provision and community-level strategies that engage community residents in community health planning. In this session, Susan Wilger, Director of Programs, will discuss their use of Prevention Institute’s Tool for Health and Resilience in Vulnerable Environments (THRIVE) framework to engage stakeholders in identifying health and safety priorities and influencing local decision making; their multifaceted approach to improving food access across the counties; the establishment of a Center for Health Innovation that is designed to assist communities in addressing social determinants of health, deepen and disseminate the workforce, and impact policy change.

This presentation is intended as part of a full session facilitated by Prevention Institute that will explore how clinical care and community prevention integration is developing across the country through specific models, frameworks and practices on the ground.
