New Orleans Health Department  staff will discuss how, a year after publishing the Department's Community Health Assessment (CHA) and Community Health Improvement Plan (CHIP), they continue to work with community partners to revisit and revise the CHIP, including thoughts on recruiting new and strategic partners, using the CHA and CHIP to inform the Department’s population health work, incorporating social determinants of health into the CHIP, working within the changing context of local politics and funding opportunities, and keeping partners motivated throughout the revision process.  This discussion will be in the context of post the Public Health Accreditation Board accreditation site visit.

Qualified on the content I am responsible for because: I am the Manager of Strategic Performance and Partnerships at the New Orleans Health Department. I was involved in the development of the department’s most recent community health improvement plan and currently provide guidance and oversight to community health improvement efforts within the department. I also serve as Accreditation Coordinator. My professional interests include collective impact approaches to reduce health disparities and address social determinants of health.
